This holds the camshaft's rear needle bearing in 750cc Ural engines from 2010 through 2022.
Why replace it? Because you want to replace the bearing. You can also remove this from the crankcase and then pull the bearing, install a new bearing, and drive the assembly back into the crankcase. Which is what I've had to do when this part is unavailable.
When you drive this into the crankcase, you need to apply sealant around it, otherwise there's a risk of engine oil dribbling out the back of the engine.
